Funding Checklist

  • An electronic copy of the executed trust documents.
  • Client's signed Social Security card (some exceptions apply) with legal name consistent on trust documents and payee line for all incoming funds.
  • If funding Medicare Set-Aside Accounts, please include "MSA" on the check memo line for payments.

To fund the following:

  • First-Party SNT
  • Third-Party SNT
  • Settlement Management Trusts
  • Minors Trust

Medicare Set-Aside (MSA) Funding Instructions

To ensure proper processing and tracking of Medicare Set-Aside (MSA) funds, all MSA funding must first be sent to CPT Institute. CPT Institute will then disburse the funds to the designated MSA administration company selected for the claimant.

  • Make checks payable to:

Important Funding Notes

  • Please ensure the claimant’s full legal name is included exactly as it appears on settlement documents. 
  • The memo line must clearly identify the selected MSA administrator to avoid processing delays. 
  • CPT Institute acts solely as the trustee/funding intermediary for proper routing of MSA funds. 
  • Funds are forwarded to the selected MSA administration company upon receipt and verification. 
  • For overnight delivery or ACH/wire instructions, please contact CPT Institute directly.

Common Funding Q&A

  • The price for the one-time trust set-up and administration are withdrawn from the trust upon funding.
  • Wire and Automated Clearing House (ACH) Direct Deposit are available but require trust documents on file before the request.
  • Electronic copies of trust documents accepted.
