Instructions to fund your Trust and Medicare Set-Aside Account
Required Items Prior To Accepting Funding
Electronic copy of the executed trust documents.
A copy of the client's signed Social Security card (some exceptions apply) . Note: The client's name on the trust documents must match their Social Security Card. Please contact CPT if this is an issue.
All funds must be sent directly to the custody agent designated below. Do not mail funds to our offices.
Failure to complete the steps above will result in funding being returned. Important Note: Trust setup and administration fees will be taken from trust corpus upon funding. There is no need to send separate payment.
Immediate singing and funding require unique funding instructions. Please inform your assigned Trust Officer if this applies.
Reminder: All checks and annuity payments must be titled the same. No fee payment required in advance. The trust setup fees and the first year’s administration fee will be taken from trust seed money. Please make sure that the name on the payee line match the client's Social Security card.
CALIFORNIA POOLED SPECIAL NEEDS TRUST BENEFICIARIES
